Paradoxical Vocal Fold Motion (Vocal Fold Dysfunction) or Episodic Dyspnea
Paradoxical Vocal Fold Motion
Paradoxical vocal fold motion (PVFM) accurately describes the abnormal and inappropriate pattern of vocal fold adduction during inspiration, resulting in inhalatory stridor.3'13'143-145 The condi- tion is primarily a respiratory issue, but the disorder is commonly evaluated and treated behaviorally in voice disorders clinics, even though dysphonia is not the primary or most distressing issue for the patient.
In severe cases, the disorder may result in chronic dyspnea, requiring trache- ostomy.3 The etiology for paradoxical vocal fold motion is unknown, but the disorder is common in adolescents, elite athletes, health care workers, and high achievers.
